Minor consent Form

All minors will need a notary form and an I.D. like a (School ID, Passport, or Birth Certificate).

Minors under 18 years of age must have a notarized consent form signed by their parent or legal guardian.

Minors under 16 years of age must be accompanied by a parent or legal guardian.
